AN INDEPENDENT review is set to be held into the death of Cerys Yemm after she was killed at an Argoed hostel last November.

Health Inspectorate Wales (HIW) will review the care, medical history and events surrounding the death of Ms Yemm at the Sirhowy Arms, with the results expected in early 2016.

Her alleged attacker, Matthew Williams, 34, who had recently been released from prison, also died at the scene shortly after police discharged a 50,000-volt Taser and arrested him.

HIW is to undertake an independent review of Mr Williams, who was known to mental health services at Aneurin Bevan, Abertawe Bro Morgannwg and Cardiff and Vale University Health Boards.

An action plan will be drawn up 'to ensure lessons are learnt from this case'.

In the review the HIW will:

Consider the care provided to Mr Willaims as far back as his first contact with health and social care services 'to provide an understanding and background to the fatal incident that occurred on November 6'.

Review the decisions made in relation to the care of Mr Williams

Consider the effectiveness of multi-agency interfaces and any potential barriers to effective partnership working in the provision of care for Mr Williams

Identify any change or changes in Mr Williams' behaviour and presentation and evaluate the adequacy of any related risk assessments and actions taken leading up to the incident that occurred 6 November 2015

Consider any other matters that may be relevant to the purposes of the review .

As part of this exercise consideration will be given also to the personal history of Mr Williams.

HIW will report upon its findings and where appropriate make any recommendations to ensure any necessary improvements in relation to the quality and safety of care are made.

The review team will seek the expertise of Care and Social Services Inspectorate for Wales (CSSIW) for matters relating to social and non-health care in Wales.

The review will commence in spring of 2015 and the final report will be published in early 2016.

Inquests into Williams's and Miss Yemm's deaths are due to be held in the autumn.
